What happens if an airplane takes off on the runway without ATC clearance?

Okay, I wonder what would happen if an airplane pilot pulls up the aircraft's throttle and then takes off from the runway without Air Traffic Control clearance. Could the pilot get in trouble? Will the flight attendants be in trouble too? Or will military aircraft be dispatched to shoot down the plane that has taken off from the airport with ATC clearance? I'm not a real pilot, so I don't know much about it, so yeah.

Assuming he does not collide with another plane taking off or landing, he will get a complaint filed against him and depending on the situation he could be grounded.

Well, first of all they could crash into another plane on the runway.

If they don't crash and they successfully get airborne, the pilot would have his license suspended while there's an investigation, and very likely would be permanently revoked.

They are not going to shoot down a commercial aircraft with civilian passengers simply because the pilot took off without clearances.

I took off in my plane last night without ATC clearance. In fact, almost all of my flights have been without ATC clearance… Because it's not required at the 90% or more of all airports that don't have a control tower.

If you take off without clearance at an airport that does have an operating control tower, you will either be asked to telephone the tower later to discuss a possible "pilot deviation", or the FAA may start an investigation. Most airports are too small for air traffic control and some only have part-time ATC service. At those airports pilots tune to the Advisory Frequency and announce their position and intentions to all other aircraft in vicinity of the airport.

At the airport where there's Air Traffic Control coverage, the tower controller will first order the aircraft attempting to take off without clearance to abort the take off. If the plane continues the take off the controller will then direct the pilot to make a phone call to the FAA to start disciplinary proceedings, up to, and including revocation of pilot's license.
