Mistakes can occur anywhere. I'm not sure what you're really asking.
Short answer: humans are involved
Long answer: People plug in the data days to weeks in advance. Fares are updated automatically based on several factors such as available seats or time to departure. An incorrect data input might only take effect once a certain threshold or set of conditions has been crossed. Mistske fares are very rare but they do happen. There's no way to predict when they will happen.
Most are just discovered by people checking fares or buying tickets. Mistake fares are usually only between a certain set of cities so its not gonna be a system wide thing. It would only benefit you if you were planning on going between two cities on a certain set of dates.
While most airlines do honor mistake fares, some airlines will cancel the tickets.

- I cancelled a flight on expedia and was refunded in airline credit. I had purchased insurance on the ticket and didn't realize I had to File a claim to be refunded. I'm **** out of luck because I already ordered my other tickets and American Airlines is charging a $200 fee on the airline credit, so even if I eventually use the credit in the next 12 months, I'm still out $200.
- First time booking through trip.com and my flights are with Scoot and Cebu Pacific Airlines. I'm unsure where I can checkin and put add ons? Is there an option to check in and add some add ons through trip.com or directly with Scoot and Ceb Pac? However, I tried managing my bookings separately on their respective websites but they both say they can't find my booking or whatever… My flight is still in a month but I d like to worry about it now than when it's nearer and I doubt they won't appear there because I've had flights before that I was able to add to my manage bookings months ahead.
