By US standards, what would be required for an airport to accommodate international flights on a regular basis?

A large enough airport to handle the traffic and people.

Terminal facilities for large aircraft
Runway of sufficient length
Demand for international routes

CBP agents

US Government would have to agree to place CBP agents in that airport. Which means a separate baggage claim area. And a lot of space.

Customs and Immigration Facilities.
It does not need to be a large Airport. Small planes make International flights too.
It does not even need to be staffed continuously. Agents can go to Airport when needed.

Airport of Entry

All civil, private aircraft entering the US must first land at an airport of entry before continuing to their destinations, unless other arrangements are made with US CBP. Advance notification must be provided electronically to CBP by means of the eAPIS. See the APIS section for more information.

Customs will expect aircraft to land at the arrival time entered on their flight plan. Arriving up to 10 minutes late is acceptable. Passengers and crew should remain with the aircraft until a Customs official arrives and be prepared to show valid documents for persons and aircraft.

Some aircraft arriving from foreign locations south of the United States must land for Customs processing at the nearest airport to the border or coastline crossing point, unless an overflight exemption has been granted (CFR Title 19, 122.24).

If it is a regular scheduled flight. Then the Airline has arranged for Agents to be available to deal with the flight. The more flights the Airport has the more space and Agents that need to be provided for clearance purposes.

Airports in USA can accept flights from pre cleared flights without having any Customs or Immigration facilities at the US Airport. That is the one of the reasons for pre clearance.
