Why would they care?

They may ask you to remove your mask when checking identity but that's it.
I think the other passengers may be the ones who are worried about catching something from you.

People in Japan travel that way all the time. It wouldn't be a problem going through the screening machines. They may want to see your face to check it against your id. It would help if you had a letter from your doctor, the way people with metal pins in them do.

They would not care.

Not a problem. They may ask you to pull it down when they are checking your ID. Otherwise, you should be able to wear it with no issue.

Just pull it down when they check your ID

They'd ask if you needed the mask for medical reasons - you'd say yes.

You'd be required to pull down your mask during identification.
